B. How often will the pen be inspected by the owner or designee? This shall include the fence, maintenance, repair and replacement of the fence, keeping the fence and any clearance along either side of the fence clear. How will this be documented?*

C. How do you plan on recapturing any escaped and farm-raised alternative livestock that may escape from or wild white-tailed deer that may enter the farm through a breach or opening in the enclosure system or fence?*

D. How were all white-tailed deer removed from the farm prior to completion of the enclosure of the farm?*

E. How do you plan on controlling the farm-raised alternative livestock population?*

F. How do you plan on identifying all farm-raised alternative livestock? Include types, timing, and method of handling.*

G. In the event that you plan to cease or are required to cease operations, what are the steps you plan to take to remove all farm-raised alternative livestock from the facility? Include notifications, removal of animals and a general timeline.*

I agree to operate this farm in conformity with Part 1 of Chapter 19-A of Title 3 of the Louisiana Revised Statues, Louisiana Board of Animal Health Regulations, the written farm operation plan submitted to and approved by the Department and any quarantine. I understand this license will be valid from July 1, through June 30, and must be renewed each year. I understand that any and/or all licenses or permits may be suspended, revoked, or placed on probation by the Board of Animal Health for any and/or all violations of the Louisiana Alternative Livestock laws or rules and regulations of the Board of Animal Health.
